Worked scenario: an assistant finishes a prophylaxis, wipes the reusable mouth mirror with an intermediate-level surface disinfectant, and returns it to the setup tray for the next patient. The plausible mistake is treating 'I cleaned it' as sufficient. The better decision is to send the mirror for heat sterilization, because a mouth mirror contacts mucous membranes and is therefore semi-critical. Why it matters: surface disinfectants are formulated for environmental surfaces, not for item reprocessing, and choosing the wrong processing level for a semi-critical item is precisely the kind of distinction scenario items are built to test.

Elongation or Foreshortening: Reading and Correcting Radiographic Distortions

Name the distortion before choosing a correction. Elongation stretches the image beyond true length; foreshortening shortens it. In a bisecting technique, insufficient vertical angulation elongates and excessive vertical angulation foreshortens.

Worked scenario: on a bisecting-technique periapical image of the maxillary molars, the teeth appear longer than they truly are and the apices are clipped off. A student reasons, 'The teeth look too long, so I should reduce the vertical angulation.' That is the plausible mistake—responding to the appearance of length by shrinking it. The better decision is the opposite: elongation signals that the vertical angulation was insufficient, so the correction is to increase it, or better, to switch to a paralleling technique with a beam alignment device so angulation is set geometrically rather than estimated. Correcting in the wrong direction turns one distortion into the other and wastes the receptor and the patient's exposure.

Train the pair with a direction drill. For each of the four distortions below, cover the correction column, name the error from the appearance, and state which way the setting moves. Then reverse it: given a correction ('I increased vertical angulation and it got worse'), reason backward to what the original error must have been. Tooth Numbering and Surface Terminology: One Rule Instead of a memorized Grid

Anchor Universal numbering on a fixed starting point: permanent teeth run 1 through 32 beginning at the maxillary right third molar; primary teeth take letters A through T on the same path. Attach surface terms to direction, not appearance.

The most common numbering error is directional: starting the count from the wrong quadrant or reversing direction at the midline. Eliminate it with the fixed rule. In the Universal system, tooth 1 is the maxillary right third molar and the count proceeds across the maxilla to tooth 16 at the maxillary left third molar, drops to the mandibular left third molar as 17, and continues around to 32 at the mandibular right third molar. Primary letters follow the identical path with A through T. Self-check: state the number of the maxillary left central incisor (9) and the letter of the mandibular right second primary molar (T) instantly—if you had to reconstruct the path, run the path aloud three times until it is automatic.

Surface terms follow the same principle of direction over appearance. Mesial means toward the midline and distal means away from it, regardless of which tooth you are describing. Buccal (or facial) is the outer surface toward the cheek or lip, and lingual is the inner surface toward the tongue. Occlusal describes the chewing surface of posterior teeth; incisal describes the biting edge of anterior teeth. Use a direction test for any scenario naming a surface: 'Which way is the midline, and which side of this tooth faces it?' That single question resolves mesial-versus-distal on any tooth in either arch.
